Who must use the Vietnam work permit application form?
In practice, the employer (company/organization in Vietnam) uses this form and submits it to the competent authority (often the provincial Department of Labor or a central-level authority, depending on the case and locality).

A foreign employee is coming to Vietnam to work for the first time (new permit)
A foreign employee is continuing employment after the current work permit expires (extension/renewal)

For the “To:” line, employers typically write either:
The Ministry-level authority (MOLISA / employment-related unit), or
The provincial Department of Labor (DOLISA) in the province/city where the employer is registered/where the employee will work
The correct “To:” line can vary by province and submission channel, so we usually confirm it based on the employer’s registered address and the file route.
Part 1: Employer/company information
Fill this section exactly as shown on the company’s official documents.
Company/organization name: Use the full legal name as stated on the business registration certificate.
Type of organization: Select the correct type (state-owned, FDI, private/domestic, etc.).
Total number of employees: Enter the accurate headcount.
Number of foreign employees: Enter the total number of foreign employees currently working for the employer.
Head office address: Match the address on the business registration.
Phone & email: Use the employer’s working contact details.
Business registration / license details: Copy the number and relevant info exactly as shown on the certificate.
Contact person for the file (phone/email): This is extremely important in real processing. The case officer often calls/email to verify or request supplements—use a reachable number and a monitored inbox.
Who can sign Form 11/PLI on behalf of the company?
Usually:
The legal representative of the company, or
A person authorized in writing by the legal representative (with proper signature/seal and clearly stated authority scope)
How many portrait photos are required for a work permit file?
Many work permit dossiers request two (02) portrait photos and commonly follow requirements such as size 4×6 cm, white background, front-facing, taken recently—always check local submission instructions because provinces can be strict on photo specs.
How long is a Vietnam work permit valid?
A Vietnam work permit is generally issued with a validity period not exceeding 2 years, based on the relevant case basis (labor contract term, assignment term, agreements, etc.).
