Instructions for Filling Out Form NA5 Visa Application and Temporary Residence Extension
Section I: Fill in Information of the Foreigner Requiring Temporary Residence Extension in Vietnam
1- Fill in the complete full name of the foreigner in CAPITAL LETTERS (example: WANG DIDI)
2- Check the gender box for Male or Female
3- Fill in accurately according to valid identification documents such as passport, ID card/Citizen ID, temporary residence card, etc.
4- Original nationality must be filled in accurately according to international travel documents such as passport
5- Current nationality must be filled in accurately according to international travel documents such as passport
6- Fill in information about the foreigner’s current occupation.
7- Fill in the passport number/international travel document number of the foreigner
Clearly specify in the “type” section whether it is an ordinary passport/diplomatic passport or international travel document
Fill in information about the authority that issued the passport to the foreigner and the passport expiration date
8- Fill in accurately the time the foreigner entered Vietnam, through which border gate, and for what purpose?
9- Fill in the period during which the foreigner is permitted temporary residence (until which date)
Clearly specify the temporary residence address of the foreigner in Vietnam
Fill in complete information related to the foreigner’s contact phone number/email

Section II – Fill in Information of the Agency/Organization or Relatives in Vietnam Inviting and Sponsoring the Foreigner:
1- Name of agency or organization (If it is an agency or organization sponsoring the foreigner requesting temporary residence extension, fill in this field; otherwise leave blank)
Address: Fill in the address of the agency or organization sponsoring the foreigner
Contact telephone/Email: Fill in contact information of the agency or organization sponsoring the foreigner
2- Full name of sponsoring relative (If it is a relative of the foreigner providing sponsorship, leave field number 1 blank and fill in information in this field).

Important Notes When Filling Out Form NA5
Each person must complete one form, accompanied by a passport or international travel document and 01 photo (size 4x6cm, white background, facing straight ahead, no glasses or accessories) and submit directly to the immigration management authority.
Must submit accompanying documents proving the relationship if sponsored by relatives of the foreigner.
For agencies and organizations inviting and sponsoring, or foreigners with temporary residence cards inviting and sponsoring foreigners, the head of the agency or organization must confirm.
For Vietnamese citizens or foreigners with permanent residence cards inviting and sponsoring foreigners, the Head of Ward/Commune Police must confirm the contents in point 2 of Section II.
